MTFC INSTITUTE – LIABILITY & LEGAL AGREEMENT

Participation, Liability & Medical Authorization Agreement

By enrolling my child in the MTFC Institute Summer Theatre Camp, I acknowledge and agree to the following:

1. Assumption of Risk

I understand that participation in theatre camp activities—including movement, acting exercises, and stage-related activities—may involve physical activity. I voluntarily assume all risks associated with my child’s participation.

2. Release of Liability

I release and hold harmless the Myles Theatre, Film & Cultural Institute (MTFC Institute), its directors, instructors, staff, volunteers, and partnering facilities from any and all claims, liabilities, damages, or expenses arising from injury, illness, or incidents that may occur during participation in the program.

3. Medical Authorization

In the event of an emergency, I authorize MTFC Institute staff to seek medical treatment for my child. I understand that I am financially responsible for any medical services rendered.

4. Health Disclosure

I confirm that I have disclosed all relevant medical conditions, allergies, and medications. I understand that failure to provide accurate information may affect my child’s safety and participation.

5. Conduct & Dismissal Policy

I understand that students are expected to follow all rules, respect staff and peers, and participate appropriately. MTFC Institute reserves the right to dismiss any student whose behavior is disruptive or unsafe. No refunds will be issued in such cases.

6. Attendance & Participation

I understand that consistent attendance is strongly encouraged, especially for participation in the final performance. MTFC Institute is not responsible for missed instruction due to absences.

7. Media Release

I grant permission for MTFC Institute to photograph and/or record my child during camp activities for educational, promotional, and marketing purposes.

8. Personal Belongings

I understand that MTFC Institute is not responsible for lost, stolen, or damaged personal items.

9. Payment Policy

I acknowledge that the $25 registration fee is non-refundable. Tuition payments are subject to MTFC Institute’s policies and are due as outlined during registration.

10. Agreement & Acknowledgment

By completing this full registration form, I confirm that I have read, understood, and agree to all terms and conditions outlined above.
