Declaration of Agreement

Participation & Conduct

I understand that participation in MTFC Institute programs requires respectful, responsible, and cooperative behavior. I agree to engage positively with instructors, staff, and fellow participants and to contribute to a safe and supportive environment.

Program Engagement

I acknowledge that MTFC Institute provides educational and creative programming designed to support personal growth, skill development, and community engagement. I agree to actively participate and follow all program guidelines and instructions.

Media Release

I grant permission to MTFC Institute to capture and use photographs, video recordings, and other media of my participation for promotional, educational, and organizational purposes, including but not limited to social media, marketing materials, and publications. (If participant is a minor, parent/guardian consent is required.)

Liability Waiver

I understand that participation in activities such as acting, movement, and performance involves a level of physical and emotional engagement. I agree to participate at my own discretion and release MTFC Institute, its staff, and partners from liability for any injuries or incidents that may occur during participation, except in cases of gross negligence.

Commitment to Safety

I agree to follow all safety guidelines and instructions provided by MTFC Institute staff to ensure a safe and positive experience for all participants.

Program Changes

I understand that MTFC Institute reserves the right to modify program schedules, instructors, or activities as needed to ensure program quality and effectiveness.

Code of Respect

MTFC Institute maintains a zero-tolerance policy for inappropriate behavior, including harassment, bullying, or discrimination.
